PLAN #1

Upper Rehabilitation - This treatment will include your 6 front teeth with your choice of materials:

• Rehabilitation with zirconium crowns - $2300 dlls
• Rehabilitation with low porcelain fusion crowns - $1800 dlls

PLAN #2

Upper and Lower Rehabilitation - this treatment will include your 6 upper and lower front teeth with the following choice of materials:

• Rehabilitation with zirconium crowns - $4600 dlls
• Rehabilitation with low porcelain fusion crowns - $3600 dlls

PLAN #3

Full Mouth Rehabilitation - This treatment will include up to 26 units of teeth. With this treatment we will use the best bio-compatible material on your back teeth so you will have a good adjustment of your bite. If you have some extractions that need to be done, as part of your procedure they will be covered with temporary crowns for the next 3 months. With those areas of extraction you have the option of implants. Please note implants are not covered in this plan.

• Full mouth rehabilitation with zirconium crowns - $9600 dlls
• Full mouth rehabilitation with low porcelain fusion crowns - $7500 dlls
